What is a risk assessment in aged care?

Our expert team develops personalised Care Plans tailored to your needs for the best possible care. A comprehensive geriatric assessment evaluates your health risks and living situation and how these factors may impact your safety at home. This proactive approach allows us to identify potential issues early, fostering stability, independence, and peace of mind.

Services you could be eligible for:

  • Fall risk assessment and installation of safety measures such as grab bars, railings, and adequate lighting.
  • Medication reviews to prevent adverse drug interactions and events.
  • Assistance in developing emergency plans for fire, natural disasters, and power outages.
  • Home safety assessments, including trip hazard evaluations, smoke detector checks, and safe storage of household chemicals.
  • Reviews of cognitive impairment issues that could lead to unsafe behaviours, such as wandering.
  • Analysis of nutrition and eating difficulties to prevent choking hazards.
Identification of social isolation risks and connection to resources that reduce these risks.
  • Ongoing reassessments after changes in physical condition or environment.

Who is eligible for these resources?

Our comprehensive risk assessments are designed for older adults receiving Home Care Package funding who wish to maintain their independence safely. We aim to help older adults identify and plan for risks early on, so they can continue living safely at home. This service is particularly beneficial for:

  • Older adults with chronic conditions that pose risks such as falls, medication issues, and nutrition deficits.
  • Individuals with reduced mobility who need assistive equipment, home modifications, and improved accessibility.
  • Older people experiencing cognitive impairment, requiring evaluations of judgment, decision-making, and safety concerns like wandering.

FAQs

We’ve put together a list of frequently asked questions to help guide you through the Home Care Package process. We know it can be confusing, but we hope this helps to explain some of the most commonly confusing topics.

A risk assessment tool is essentially a comprehensive checklist. It covers a thorough inspection of the home environment, conducted by a specially trained assessor, to identify safety hazards and vulnerabilities. This home safety assessment examines the places you frequent, your abilities and physical challenges, medication usage, eating and hygiene patterns, cognitive impairment issues, social support networks, and emergency systems. The assessor may also discuss lifestyle, nutrition, and medical status to gain greater context and provide recommendations.

A safety check helps prevent accidents and emergencies by identifying hazards and vulnerabilities early on. Using a comprehensive risk assessment tool that examines physical conditions, abilities, home safety, medications, mobility limitations, and other factors, care teams can pinpoint risks that could lead to serious incidents if left unaddressed.

If a significant hazard is identified, the assessor documents the findings in a hazard report. High-priority risks require immediate control measures across the care team. For example, if the stove poses a danger due to client confusion, the team might install a stove timer device to prevent unattended use until cognition improves through special support services outlined in the plan.

Risk assessments should occur:

  • Upon initial enrolment in-home care.
  • Annually, as routine monitoring.
  • After a fall or hospitalisation.
  • After a major decline in physical condition.
  • Upon a change in environment, such as moving homes.
  • After natural disasters that cause damage that requires renovations.
  • Regular assessments help address new risks as your well-being and abilities change over time.
