At Lucky Care, we have the largest list of service providers in Australia, which is growing every day. We don’t lock you into using our staff or make you pay inflated rates; instead, we broker workers nationally. This gives you the freedom to choose and find care workers who are right for your situation and are local to you.
Your provider is required to supply you with a written Care Plan. A Care Plan is designed to meet your goals and assessed care needs as determined by your aged care assessment services. Your provider will put this Care Plan together in consultation with you during your initial meeting. It will explain in detail the needs and services required to remain at home. You are required to be issued with this Care Plan within 14 days of entering into your agreement.
When planning your care, consider whether you prefer a public or private hospital. Each option offers different benefits, such as wait times, availability of specialists, and out-of-pocket costs. Your choice will impact the discharge process and the type of transition care program you may need after leaving the hospital.
Yes, post-hospital care can be coordinated through a community health centre. These centres often work closely with your transition care provider and hospital healthcare team to ensure you receive the necessary support to recover effectively at home.
