What’s included in Depression Care for Older Adults?

Our depression care services are designed to improve your life through a comprehensive, multidisciplinary approach that addresses the emotional and social aspects of your well-being.

Depending on your Home Care Package, you may be eligible for the following:

  • A medical examination of your mental health to identify symptoms of major depressive disorder or other mental disorders that may affect your overall health.
  • Assessment by a mental health professional to evaluate the severity and nature of major depression in older individuals like yourself.
  • Encouragement and support to build a strong support system through social connections, reducing isolation and enhancing your mental well-being.
  • Tailored treatment options for depressive disorders, including those with severe depression.
  • Guidance on nutrition and medication to support both your mental and physical health, especially if you’re dealing with mood disorders.
  • A depression nursing care plan specifically designed to help manage depressed mood and improve your quality of life.

Who would benefit from our mental health services?

These services are particularly beneficial if you’re showing signs of depressive disorders or living with major depressive disorder. Here’s how you might benefit:

  • If you’re experiencing persistent feelings of sadness, hopelessness, or depressed mood over an extended period, these could be signs of major depression.
  • If you’ve lost interest in activities you once enjoyed, such as socialising or hobbies, this could be an indicator of mood disorders that need attention.
  • If you’re having trouble sleeping — whether it’s difficulty falling asleep, staying asleep, or oversleeping — this might be a symptom of a mood disorder.
  • If you find yourself avoiding social activities or withdrawing from relationships, this behaviour could increase your suicide risk.
  • If you have chronic health conditions, pain, or disabilities, you may be at a higher risk of developing severe depression, and our nursing care can help focus on both your mental and physical health.

FAQs

We’ve put together a list of frequently asked questions to help guide you through the Home Care Package process. We know it can be confusing, but we hope this helps to explain some of the most commonly confusing topics.

A nursing diagnosis is a critical step in depression care, as it involves assessing and identifying the specific needs and challenges you face. This diagnosis helps to tailor a depression nursing care plan that addresses both your physical and emotional health, ensuring comprehensive support for those experiencing major depressive disorder or other mood disorders.

If there’s a concern about self-harm, our care team works closely with mental health professionals to develop a safety plan for you. This may include close monitoring, regular mental health check-ins, and creating a supportive environment to reduce suicide risk. We also ensure you have access to a strong support system to help you navigate through difficult times.

A medical illness can significantly contribute to or exacerbate depression. Conditions such as chronic pain, heart disease, or diabetes can increase the risk of developing depressive disorders. Your care team considers these factors when developing a nursing diagnosis and treatment plan, ensuring that both your mental and physical health needs are addressed holistically.

You’re not obligated to stay with a provider if you’re not satisfied with their services. The Consumer Directed Care (CDC) legislation, amended in February 2017, empowers Home Care Package holders with the freedom to actively participate in their care. If you’re happy with your current provider, that’s great! If not, you have the option to switch to a provider that better suits your needs. Just check your current Home Care Agreement for any fees or special conditions, including the notice period required to leave your provider.
