Learn more about our Carlton Home Care Package services.
Home Care Packages (HCPs) are intended to support older Australians to live independently in their own homes by funding necessary aged care services.
However, there are specific services that are not covered by the Packages such as:
- Basic daily living expenses that all residents typically manage independently.
- Refundable accommodation deposit paid for entry into residential aged care facilities.
- Items of personal comfort and convenience and general household items that do not directly relate to care needs.
- Costs associated with accommodation.
- Fees for home care services not included in the Care Plan or not approved by your Care Manager.
- Travel and accommodation for holidays.
- Gambling and illegal activities.
Residential aged care and home care are two distinct types of support offered to elderly individuals, each designed to cater to different needs and preferences.
Residential care involves moving into a facility where 24-hour care is provided. Residential aged care is suitable for individuals who require constant care and supervision that cannot be easily provided at home, or for those whose caregivers can no longer meet their increasing care needs.
Home care allows individuals to remain in their own homes while receiving home care services based on their specific needs. Services can range from light tasks in your house and shopping assistance to nursing and physical therapy. Home care is highly customisable, designed to assist older people in living independently while still receiving the necessary aged care support. This option is ideal for those who need some assistance but wish to stay in their familiar environment for as long as possible.
Both options have their benefits, and the choice depends largely on the individual’s care needs, personal preferences, and overall health condition.
Your Provider will be required to supply you with a written Care Plan. A Care Plan is designed to meet your goals and assessed care needs as determined by your Aged Care Assessment Team (ACAT) assessment. Your Provider will put this Care Plan together in consultation with you. It will explain in detail the needs and services of a client.
The Care Plan will set out goals and services you need to remain at home. You are required to be issued with this Care Plan within 14 days of entering into your Home Care Agreement.
The Care Plan is a type of guide for what products and services you can organise to best support you.
