Have questions about home care support in Brisbane?

When you are assessed for a Home Care Package, the assessor will also establish what level of funding you may be eligible for. There are one of four levels of Home Care Packages you may be eligible for.

Level 1.

A Level 1 Home Care Package is designed to cover basic care needs. This can include basic everyday tasks that you may require assistance with. Depending on which Provider you choose, you will have a different amount of funding available.

Level 2.

A Level 2 Home Care Package is designed to support an individual with low-level care needs. This package provides more funding than Level 1. The funding can help older Australians receive support with a range of tasks.

Level 3.

A Level 3 Home Care Package is designed to support those older Australians with intermediate care needs. With this package you are able to choose from a range of support and services to help you remain living at home.

Level 4.

The Level 4 Home Care Package is the highest level. It is designed to support older Australians with high care needs. With a Level 4 package, you are eligible to receive more than $60,000 per year. These funds are designed to help you continue living in your own home while receiving complex care.

A Referral Code is a unique number (1- number) that you receive when you are assigned a Home Care Package. This code allows the Provider of your choice to access your ACAT/ACAS assessment which contains important details about your situation and your unique requirements. This code is also important as your approved provider will use it in order to trigger your Home Care Package funding on your behalf.

The Commonwealth Home Support Programme (CHSP) is designed for seniors who need basic assistance with everyday tasks, such as cleaning, shopping, or transport. It offers entry-level support, and you usually pay a subsidised fee for your services.

Home Care Packages (HCP), on the other hand, provide more comprehensive and flexible care for those with higher or more complex needs. HCP funding can cover many services, including personal care, nursing, and home modifications, giving you greater control over your care plan. However, depending on your income, you may be required to pay an income-tested care fee, which is calculated based on your financial situation.

Both programs aim to support your independence, but the right choice depends on the level of care you need.

During your in-home assessment visit, the ACAT/ACAS assessor (usually nurses, social workers, or other allied health professionals) will ask you questions about how well you are managing your day-to-day life, your individual needs, and your financial situation. They will also give you some advice about the different types of in-home care services that may help you stay at home and maintain your independence.

This assessment will determine if you are eligible for a Home Care Package and, if so, at which level. You can have a family member or advocate present at the assessment.

Several factors determine eligibility for Home Care Packages in Australia. Primarily, older Australians who need assistance to live independently at home can qualify for these Packages depending on their unique health needs. The Australian government provides these Home Care Packages through government funding to support older individuals with different services for varying care needs. These packages include various care services aligned to clients’ Care Plans. The care and support they could receive from their Home Care Provider includes personal care, disability support services, domestic assistance, and clinical services, tailored to meet individual support requirements.
