What’s included in Care Management?

Care Management is an essential service for all Home Care Package recipients. It involves the consistent evaluation of a client’s needs, objectives, and preferences by a Care Manager. It includes the regular review of their Home Care Package and Care Plan, ensuring that the care and services provided are aligned with other available supports. The Care Management services you receive depend on the Provider you sign with and the services they offer.

Services you can receive through Care Management:

  • A dedicated Care Manager who provides continual management and review of the care recipient’s home care agreement and Care Plan to ensure they align with their changing needs and preferences.
  • Management of the financial aspects of your Home Care Package, ensuring that the allocated budget is optimally utilised to meet your needs and that your funds are used to their fullest potential.
  • Maintenance of accurate records, generating reports on the Care Management process, ensuring transparency and facilitating informed decision-making expertise.
  • A dedicated Care Manager who will manage the coordination of services outlined in your Care Plan, prioritise your safety, and handle necessary paperwork for comprehensive and efficient care delivery.

Who is Care Management for?

Every Home Care Package recipient receives Care Management services through their package. Providers are required to offer these services if they are chosen to manage a recipient’s package. You decide on who you would like to offer this service and manage your Package. It’s important for you and your family to be aware that different Providers offer different services. You can discuss the degree of support you would like from a Care Manager and decide on your management option:

These services are specifically designed for:

  • If you choose to self-manage, you will still have Care Management support. If you choose to self-manage you will take control of the day to day managing of your Home Care Package, but still have a range of duties completed by your Care Manager.

FAQs

We’ve put together a list of frequently asked questions to help guide you through the Home Care Package process. We know it can be confusing, but we hope this helps to explain some of the most commonly confusing topics.

A Home Care Package (HCP) is funding given to older Australians through a government-subsidised program to provide long-term support for elderly individuals who want to continue living independently at home.

At Lucky Care we have the largest list of service providers in Australia which is growing every day. At Lucky Care we don’t lock you into using our staff at inflated costs, instead we broker our services nationally. This means you have the freedom to choose and find care workers right for your situation, local to you.

Each month My Aged Care will send your chosen Approved Provider your monthly government subsidy. Your funding is not paid in a lump sum for the year, but instead as a monthly payment. If you receive a Home Care Package supplement, it will be paid to your Provider each month in addition to your basic subsidy.

This fee is also called a standard resident contribution in aged care legislation and Providers can decide whether to charge this. Lucky Care doesn’t charge a Basic Daily Fee.
