Learn more about aged care services in the Docklands.

Home care and residential aged care cater to different needs and preferences. Home care allows an older person to receive personalised care and support in their own home, maintaining independence and comfort in familiar surroundings. Services can range from personal care to nursing and visiting medical services.

In contrast, residential aged care provides 24-hour support in a dedicated facility, suitable for those who need constant aged care or have complex medical needs that can’t be adequately managed at home. This option offers structured environments with access to various health and medical professionals and social activities, making it ideal for those requiring more intensive support.

To apply for a Home Care Package, start by scheduling an assessment with the Aged Care Assessment Team (ACAT). You can arrange this through the My Aged Care website or by calling them at +61 460 797979. In the initial phone call, they will ask you some questions to understand your current status. Once assessed and approved for a Package, you will be given a level of eligibility and be placed in a national queue. When a Package becomes available, you’ll receive a letter from My Aged Care, with your unique referral code which you will give to the Provider of your choice to allow them to access your funding and start your services. We are one of those Home Care Package Providers.

HCPs enhance safety at home by incorporating various services and supports tailored to individual needs. Through ensuring that you have services that support your mobility, social connection and addressing hazards within your home environment, you can stay more safe at home. The HCP not only addresses health and daily living needs but also emphasises security measures relevant to your home, helping to create a safer living environment.
