  • Allied health therapies (e.g. physiotherapy, podiatry, occupational therapy)
  • Care management and goal-setting
  • Nutritional support and meal planning

Who is clinical care for?

Our home-based clinical care services are designed for older Australians managing chronic health conditions, those recovering after hospital discharge, and anyone requiring ongoing allied health support or wound care. By providing care in the comfort of your own home, we help maintain your independence while supporting your health and wellbeing.

We also support individuals with reduced mobility or frailty, people at risk of hospitalisation or falls, and those needing end-of-life care at home. Our goal is to provide compassionate, professional care that keeps you safe, supported, and comfortable, while reducing the need for hospital visits whenever possible.

How clinical services are delivered

You’re assessed through My Aged Care and assigned a funding level.

Your funding covers eligible clinical services within your care plan.
